The app actually isn't working for everybody and Medium is not listening to comments. I went through the process, found everything, but the app update with the additional upgrade option is a dead link. I explained that to medium, and they answered with you should be eligible soon as we are rolling it out this week. Duh, I can see that I'm eligible, but the app is messed up and there aren't any updates.
